1. Purpose of consultancy
General objective:
This activity aims to generate health expenditure based on disease in the JKN scheme using the 2022 and 2023 JKN data. One selected technical assistant (TA) is expected to assist the Center for Health Financing and Decentralization Policy (Pusjak PDK) in managing/processing data, analyzing, and producing health expenditure by diseases in the JKN scheme within the agreed period.
Specific objective:
1) Provide input to adjust the Health Accounts methodology in producing higher quality and detailed health expenditure data by diseases in Indonesia.
2) Determine how health expenditure data by diseases are collected, managed, and classified to align with the Health Accounts methodology.
3) Identify the types of health expenditure data that are most useful to decision-makers in Indonesia; and early efforts to collect, analyze, and use health expenditure data by disease to meet the needs of decision-makers in the pharmaceuticals sector.
4) Conduct advanced analysis based on the JKN data.
5) Assist and support the Center for Health Financing and Decentralization Policy in conducting meetings or workshops related to Health Accounts activities and provide progress reports on these activities to the Center.
2. Background
Disease Accounts is an analysis of the distribution of health expenditure by diseases that is part of the National Health Accounts (NHA). Analysis of health expenditure by diseases can monitor and inform the allocation of resources by diseases in populations or priority areas. The analysis can provide an overview of health expenditure based on 1) diseases with the largest visits and health costs, 2) JKN participant registration and service areas, and 3) functions and service levels.
Currently, Indonesia is facing a triple burden disease of various disease problems, namely the existence of new emerging and re-emerging infectious diseases, such as COVID-19; infectious diseases that have not been resolved properly; and non-communicable diseases (NCDs) which tend to increase every year. As a result, it can be seen that Indonesia’s health expenditure continues to increase and is still focusing on curative efforts. The increase in cases of non-communicable diseases (NCDs) will significantly increase the burden on the government and the community because handling them takes a long time and requires large costs.
Disease Accounts production has used the JKN data since 2015 and continued to be updated, both data and methodology. With the JKN coverage of more than 90 percent, Disease Accounts are very representative of getting a picture of health services based on the type of disease. Disease accounts in NHA are expected to be produced annually on an ongoing basis so that they can provide benefits that can be used as input for improving health service policies.
3. Deliverables
1) Report on capacity building on disease accounts production 2022 and 2023.
2) User guide on disease accounts 2022 and 2023 production.
4. Qualifications, experience, skills and languages
Educational Qualifications:
Essential: A doctor’s degree in Public Health, particularly health economics, health financing, or health policy
Desirable: –
Experience
Essential:
1) At least two years of experience working in Health Accounts or Disease Accounts;
2) Health financing data collection and management, particularly disease accounts;
3) Data analysis.
Desirable: –
Skills/Knowledge:
1) Strong academic theoretical knowledge, expertise, and technical experience in the field of Health Accounts and Disease Accounts.
2) Have experience performing NHA estimation and Disease Accounts.
3) A good track record in previous projects with the Ministry of Health, other development partners, and other national and local governments.
Languages and level required (Basic/Intermediate/Advanced):
Intermediate level of English.
5. Location
On site: Consultant will cooperate with the Center for Health Financing and Decentralization Policy at the office of the Ministry of Health or a place regulated for Disease Accounts activities.
6. Travel

7. Remuneration and budget (travel costs are excluded):
a. Remuneration: IDR 30,000,000 monthly
b. Living expenses (A living expense is payable to on-site consultants who are internationally recruited): N/A
c. Expected duration of contract: 13 months
