Your Opportunity

As a Case Manager with the THRIVE service in Carnarvon, you will have the opportunity to make a meaningful difference for individuals and families working to sustain safe and secure housing. THRIVE supports tenants of Public Housing and Aboriginal Housing who may be experiencing complex challenges, and your role will help them build confidence, capability and stronger connections within their community.

This is an on-site position where you will work directly with clients in their homes and neighbourhoods, taking a strengths-based approach to understand their goals and the issues affecting their tenancy. You will provide practical support, consistent engagement and structured case management, linking clients with services and resources that enhance stability and wellbeing.

Most referrals come through the Department of Communities (Housing), local service providers and self-referring tenants, and you will support people from first contact through to achieving sustainable tenancies. This role offers the chance to combine empathy, problem solving and practical support in a way that has real and lasting impact.

Your Key Responsibilities

  • Conduct initial assessments, inductions and risk assessments for new referrals, ensuring suitability and clarity of support needs.
  • Develop, implement and regularly review individual case plans using a strengths-based and goal-focused approach.
  • Provide face-to-face and outreach support, including home visits, to help tenants maintain safe and stable housing.
  • Work collaboratively with the Department of Communities (Housing) and other key partners to address tenancy challenges.
  • Facilitate referrals to internal and external support services to address complex needs and strengthen client capacity.
  • Maintain accurate and timely documentation, case notes and data entry in line with Mission Australia protocols and program requirements.
  • Support clients until they transition out of the service or achieve a sustainable tenancy, ensuring continuity of care and capacity building.

Requirements for Success

  • Relevant qualifications such as a degree, associate diploma, certificate or equivalent experience suitable to Mission Australia’s requirements.
  • Emerging professional experience with a willingness to learn and grow in a structured case management environment.
  • Experience supporting clients with complex or challenging circumstances, including those facing housing instability.
  • Strong communication skills with proficiency in technology, record keeping and administrative processes.
  • High level of cultural competence, including strong understanding of Aboriginal and Torres Strait Islander peoples, histories and protocols.
  • Ability to work respectfully with vulnerable clients, maintain clear professional boundaries and support individuals in distress.
  • Excellent local knowledge of Carnarvon and strong community connections to enhance referral pathways and client outcomes.
  • First Aid and CPR Certificate.
  • Full driver licence with no restrictions

Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
